After an initial period of observation (around
three months), aimed at getting to know one another
(structural), staff make an initial valuation of
the client's problems: such an evaluation forms
the basis for the appropriate program for the individual,
bearing in mind the particular needs of each case,
their difficulties and their abilities, as well
as setting out the personalized objectives to be
pursued. The objectives are revisited every six
months in the course of the community stay and are
discussed and verified at periodic meetings with
the appropriate authorities
Specifically, the community program aims to give
each client the opportunity to adapt more easily
and achieve greater autonomy, as much as can be
permitted by the pathology and the difficulties,
through help and by following the following main
areas:
1. PRIMARY NEEDS
2. HEALTH AREA
3. RECREATIONAL GAMES AREA
4. ERGOTHERAPY AREA
5. SOCIAL RECREATION AREA
6. COGNITIVE REHABILITATION AND CULTURAL TRAINING
AREA
7. SPORTS AREA
8. THERAPEUTIC AREA
9. ASSISTANCE AREA
10. EMOTIONAL / SEXUAL AREA
11. SPIRITUAL AREA
The areas in question represent the 11 main aspects
whereby community work and life are expressed: these
are each subdivided into sub-aspects, known as "activities",
which represent in turn the tangible tools that
the team makes available to chart the "change".
From the chronological point of view, the individual
educational and / or therapeutic course essentially
coincides with the following fundamental phases
of the community program.
